Eagle Fern Veterinary Hospital, PC is pleased to provide you with forms on line that can simplify the process admitting your pet for an exam or procedure, providing your information, refilling a prescription or requesting an appointment.  Just select from the available form on the menu below.

New Client  Prescription Refill Change of Address   Files - PDF  Please click Files - PDF above to access forms as indicated below for your pet's next appointment.  We encourage you to review and fill the forms out prior to your appointment, especially if you are having someone else who is not familiar with your pet or your concerns bringing in your pet for you.   Please allow up to an hour for an appointment.  If needed, you may admit your pet in the hospital for an exam for the day. The doctor will examine your pet and inform you of their findings and cost estimate.  **In order to do this, we ask you fill out the Day Admission Authorization form as well as the other forms indicated for exam, surgery, dental procedure. If you have any questions or concerns, contact the Client Service staff or your veterinarian.  The hardest part is to remember to bring the forms with you to your appointment! DAY ADMISSION AUTHORIZATION:  Complete if you are unable to be present for your pet's examination.
